Lookup Lists

New forms were created to enable easy editing of lookup lists including the Surgeon Label, Hospital Label, Operation Label, Anaesthetist Designation, Health funds, Payers (drawers), Banks, Branches, Quick items, Quick notes and Document Types (Category of linked document). A link to the older datasheet editing format is also provided on the editing form. The forms are usually opened by selecting the <Edit Lookups> option from the appropriate drop down list, or by clicking a 'pencil' button. Some of the separate 'pencil' buttons have been removed where they were redundant.

Added an option to the Tools menu to easily open any of the lookup tables for editing, without having to go to the specific field to open the lookup table.
